Original Pakistani Passport or Pakistani ID Card (CNIC/NICOP) along with one photocopy. When you apply for a Children birth certificate from the union council, make sure that you have all the following documents in hand.
  1. Application form for a computerized birth certificate.
  2. Affidavit on stamp paper.
  3. Handwritten application to chairman union council.
  4. Birth Receipts of Hospital (known as Janam Parchi)
  5. CNIC of parents.

How to Get Birth Certificate from Union Councils

You can contact the office of the local Union Council within 60 days of the birth for registration of the birth. You will be required to produce the following documents:
  • CNIC copy of the parents
  • Birth certificate issued by the hospital or traditional birth attendant
  • Form provided by the Union Council, duly filled in along with signature and thumb impression

Birth Certificate Fee

In Punjab, birth registration is absolutely free. However, a fee of PKR. 100/- will be charged for issuance of the NADRA Computerize Birth Registration Certificate. Secretary Union Council will receive this fee.
Fee may vary in some provinces but it is not more than Rs.300 for a normal (within 60 days) birth registration anywhere in Pakistan.

You will get the certificate within 3 working days (in the case of normal registration). However, it may take 7-20 days for late registration.

You have to visit your union council or e-Khidmat Markaz because union council's birth certificate is not issued online.

How to get Birth Certificate from e-Khidmat Markaz in Punjab

e-Khidmat Markaz provides hassle-free services to the citizens under one roof in timely manner through appropriate application tracking ID from the e-Khidmat Markaz website, helpline and SMS gateways. Service are be offered with promising time management systems.
Following documents are required for issuance of birth certificate from e-Khidmat Markaz:
  • Duly filled application form A
  • Two recent photographs of child
  • Original and a copy of CNIC of applicant
  • Original and copies of CNICs of father, mother and grandfather
  • Original and a copy of Birth slip of a child issued by hospital (in case of birth in a hospital)
  • Rs.200 fee
  • Original and a copy of vaccination card or school certificate or hospital's birth certificate (after 61st day and within 7 years of the birth)
  • Attested Affidavit Rs. 50 (2 witnesses) in case of Birth in house (after 61st day and within 7 years of the birth)

Birth Certificate Fee

The birth certificate issuance fee is Rs.200 within 60 days and the certificate will be issued within 7 working days.
